Activity of Daily Living (Assistance with Self-Care Activities)
Assist clients with their daily activities which includes all support services around meeting their needs on a person centre support basis. Which also includes assisting clients with their medications, transfers, mobility support needs, personal care, grooming and other self-care activities.
Domestic Assistance Support
Provide support service including house cleaning, laundry, lawn mowing, house tidying and all other household tasks.
Community and Civic Participation Support Service
Assist clients with their community access support which includes, shopping, work support, doctor’s appointment, travel and supporting them to their activities of interests. Also provides support to clients to enable them to engage in the community.
Support Coordination Service
Support clients to build their capacity, strengthen their ability to coordinate and implement supports in their plan as well as supporting them to engage in the community. Provides support coordination services for clients by serving as an advocate for them, linking them with the right service providers and services that are appropriate to meet their support needs. Also supporting them to receive the maximum out of their current NDIS Plan and Plan Review Preparation. This could also include gathering of Plan Review Reports from Providers, providing evidence for increase funding, budget management, liaising with their NDIS Planners and their Mainstream Supports.
Support Coordinator is also very experienced and have work with many clients to assist them with their support coordination services. Support Coordinator also work with Mental Health and very complex support needs clients in the community and the hospital. Especially supporting them to meet their housing and home and living support goals.
Transport Service
Provides transportation to clients to the destination of their choice. Drivers are very experienced and kind and will support you to and from your destination.
